This is especially true when relatives live out of town. Our Care Coordination staff has the experience to help make the logistics manageable. Care Coordination staff provide assistance with the following:

• Scheduling of appointments and arranging for transportation
• Communication with other providers, such as physicians, estate planners, or private caregivers
• Arranging for upkeep of the home, such as snow and waste removal, pet care, or lawn care
• Oversight of a client's medical condition, including medication management, assessments, reporting to their physician, implementing physician orders, etc.
